Proprietary name [and dosage form]: Panamor gel. Composition: Each 100 g gel contains: Diclofenac hydroxyethylpyrrolidine - 1.292 g. Preservative: Isopropanol - 8.0% m/m. Pharmacological classification: A 3.1- antirheumatics (anti-inflammatory agents). Pharmacological action: Panamor gel is an anti-inflammatory and analgesic preparation designed for external application. Following the local application of Panamor gel, the active ingredient is absorbed through the skin. The percutaneous absorption of diclofenac, estimated in healthy volunteers by measuring the unmetabolized fraction in the urine, is about 6% of the applied dose. Experimental studies in animals have shown that after local application, the active ingredient enters the skin and penetrates the underlying tissues where it attenuates the acute and chronic inflammatory reactions. The anti-inflammatory and analgesic properties of Panamor gel in traumatic and rheumatic inflammation result in a decrease in swelling and a reduction in pain on pressure and movement. Indications: Panamor gel is indicated for the symptomatic relief of localised traumatic inflammation and pain. Identification: White to ivory-white, milky, uniform gel with a characteristic odour of isopropanol and perfume. Presentation: Red aluminium tubes of 30 g, 60 g and 100 g. How to use

Usage instructions:
Dosage and directions for use: Adults: According to the size of the affected area, apply 2 to 4 g Panamor gel (1 to 2 fingertips) 3 to 4 times daily. The gel should then be rubbed in gently or massaged into the skin in case of muscular pain. Children: Confine use to adults only, as safety and efficacy have not yet been established in children.

Warnings:
Contra-indications: Hypersensitivity to diclofenac, acetylsalicylic acid and other nonsteroidal anti-inflammatory agents, as well as isopropanol and polyoxyethylene glycol. Panamor gel should not be used by patients with porphyria. Side-effects and special precautions: Side-effects: Panamor gel is usually well tolerated. Itching, reddening, or smarting of the skin or skin rash may occur. Photosensitivity reactions have been reported. Where Panamor gel is applied to relatively large areas of skin and over a prolonged period, the possibility of systemic side-effects cannot be excluded. Side-effects as experienced with systemically absorbed diclofenac sodium include the following: Epigastric pain, eructation, nausea and diarrhoea, headache and slight dizziness. If they persist or are troublesome, the preparation must be discontinued. There have been reports of skin rash, peripheral oedema, gastrointestinal ulceration or haemorrhage, hypersensitivity reactions (e.g. bronchospasm, anaphylactic/anaphylactoid systemic reactions), elevated transaminase levels, jaundice, hepatitis, renal failure and nephrotic syndrome. Isolated cases of dyshaemopoiesis (leucopenia, thrombocytopenia, aplastic anaemia) and erythema multiforme have been observed. During prolonged treatment with Panamor gel, blood counts and monitoring of hepatic and renal function are indicated as precautionary measures. Despite the topical application of the product, hypersensitivity reactions of the skin and mucous membranes e.g. asthma, angioedema and urticaria cannot be ruled out in patients who are allergic to aspirin or certain additives. Patients suffering from asthma (intrinsic) or chronic urticaria are particularly prone to reactions. Special precautions: Panamor gel should be applied only to intact skin surfaces, and not to skin wounds or open injuries. It should not be allowed to come into contact with the eyes or with mucous membranes. Not to be taken by mouth. Interactions as experienced with systemically absorbed diclofenac sodium: When given concomitantly with lithium, non-steroidal antirheumatic agents raise the concentration of lithium in the blood. The bioavailability of Panamor gel is reduced by acetylsalicylic acid, and that of acetylsalicylic acid by Panamor gel, when the two agents are administered together. Pregnancy: The safety of Panamor gel in pregnancy has not been established. Known symptoms of overdosage and particulars of its treatment: In the event of significant systemic side-effects occurring as a result of improper use or accidental overdosage (e.g. in children), general therapeutic measures of the kind normally adopted in order to treat poisoning with non-steroidal anti-inflammatory agents should be resorted to. Treatment is symptomatic and supportive. Keep out of reach of children. For external use only.
